INTRODUCTION

The pulmonology (respiratory) services at Serdang Hospital started on 1st of November 2011 as a unit under the General Medical Department. In 2019, the unit was upgraded to a department.

The Pulmonology Department provides comprehensive services to both inpatients and outpatients. At Serdang Hospital, the pulmonologists (respiratory physicians) from both the Ministry of Health Malaysia and University Putra Malaysia (UPM) work hand in hand as a team. Patients with lung cancer, massive haemoptysis and central airway obstruction will be given priority and managed promptly.

The department serves as a training centre for Masters (internal medicine, anaesthesia and Family Medicine) MRCP, fellowship in pulmonology and interventional pulmonology in Malaysia. Specialists from abroad such as India, Indonesia and the United States of America have come to the department to receive interventional pulmonology training.

The department also serves as the National referral centre that offers bronchoscopic management to patients with asthma and emphysema (chronic obstructive pulmonary disease) and laser-assisted tumour resection in the management of lung cancer involving the airways. Beside these services, we also provide advanced lung disease services that ultimately need lung transplant such as advanced emphysaema, interstitial lung disease (ILD) and pulmonary hypertension since early 2019 and will continue to expand these in years to come.

The department collaborates with the National Heart Institute (IJN) for the National lung transplant program. Patients with advanced lung diseases who need lung transplantation are referred to the lung transplant clinic in Serdang Hospital for assessment and the lung transplant surgery is performed in IJN.

The department is the main referral centre for complex respiratory cases in Selangor. We also receive complex respiratory cases from Kuala Lumpur, Putrajaya, Malacca, Negeri Sembilan, Johor, Perak and Penang (both government and private healthcare facilities).

- Advanced emphysaema for bronchoscopic lung
volume reduction (BLVR)

♦ Bronchoscopy clinic (at Thoracic Endoscopy suite,
Daycare Unit)
♦ Lung nodule (lung cancer) clinic

♦ Pulmonary Physiology Laboratory:
♦ Providing non-invasive tests as follows:
♦ Spirometry
♦ General respiratory function test (GRFT)
♦ Bronchial Challenge test (BCT)
♦ Cardiopulmonary exercise testing (CPET)

INPATIENT SERVICES

Providing treatment for general respiratory cases such as asthma, COPD, pneumonia, tuberculosis and more advanced services (central airway obstruction and massive haemoptysis)

Patients with lung cancer are jointly managed with the Department of Cardiothoracic Surgery, National Cancer Institute (IKN), Department of Radiology and Department of Pathology.

♦ tumour debulking
♦ airway dilatation
♦ laser bronchoscopy
♦ argon plasma coagulation (APC)
♦ transbronchial needle aspiration (TBNA)
♦ airway stenting
♦ topical mitomycin C
♦ bronchoscopic lung volume reduction (BLVR)
♦ talc poudrage pleurodesis
♦ linear EBUS (endobronchial ultrasound)
♦ navigation bronchoscopy
♦ radial EBUS
♦ bronchial thermoplasty (BT)
♦ indwelling pleural catheter (IPC)

Sleep lab:
♦ Offers sleep studies, cPAP and BiPAP trial

DAYCARE UNIT

♦ Provides flexible bronchoscopy, pleural ultrasound and
pleuroscopy

♦ Receives referrals from nearby hospitals that require
flexible bronchoscopy and pleuroscopy for diagnostic
purposes.

Client Charter

To ensure each patient suspected of having lung cancer is seen in our specialist clinic no longer than 2 weeks after referral
To ensure each patient suspected of having pulmonary tuberculosis (smear positive) is seen in our specialist clinic no longer than 2 weeks after referral
To ensure each patient suspected of having obstructive sleep apnea/obesity hypoventilation syndrome/central apnea will undergo polysomnography (sleep study) within 1 month
To ensure patients who have acute central airway obstruction are managed promptly within 24 hours
To ensure patients who suffer from massive hemoptysis (severe coughing out of blood) are assessed and managed promptly within 1 hour
To ensure each patient with lung cancer who is scheduled for lung cancer resection will undergo pulmonary function testing (PFT) and/or cardiopulmonary exercise testing (CPET) within 1 week before the scheduled operation
